At Castleton Health Care Center, we don’t believe in one-size-fits-all health care. From your first care conference, our team will do everything they can to get to know you for the irreplaceable individual that you are. We want to understand your preferences and adapt our care, if or when your needs change. We also want to be able to anticipate these changes beforehand, which makes getting to know you of the utmost importance.

From the management team to each member of our hands-on staff, the experts at Castleton Health Care Center are experienced providers of powerful and dynamic clinical care. Proudly accepting patients whose challenges require specific and high-level skill sets, our scope of expertise includes, but is not limited to, long-term patients, trach patients, bariatric patients, patients with surgical or pressure wounds, patients with both new and long-term feeding tubes, TPN, CADD pumps, new amputees, and CPAP and BiPAP patients.

24 Hour Skilled Nursing Care

Cleveland Health Care Center offers short-term or long-term options to help your loved one recovery as quickly as possible so they can return home and to their day-to-day life. We offer inpatient and outpatient therapy and rehabilitation services to all families.

Following surgery, or injury or illness, our staff provides skilled nursing services, and therapy and rehabilitation your loved one needs. There are a number of reasons you may need our care including stroke, broken bone, or surgery—our facility Cleveland Health Care Center, provides individuals with the 24 hour support they need to allow for a faster and improved long term recovery.

Housekeeping/Laundry Services

We understand that the aging process or illness can strip independence away from our residents and we strive to help our residents live as independently as possible while maintaining their security and safety. We ensure that our residents remain active in their day to day life and ensure that they are involved as much as possible in their care plan.

Each resident is given the privacy of their own space to call home with tons of extra space for them to relax and enjoy themselves in. There are additional services available at the facility such as housekeeping, laundry services, transportation, a pharmacy, and more.

Wound Care

Cleveland Health Care Center offers a comprehensive range of treatments to aid in the healing of chronic wounds caused by diabetic ulcers, pressure ulcers, radiation injuries, venous insufficiency and other conditions. We provide full-service care to treat patients with emergency and chronic wounds resulting from various health conditions.

Physician, Physician Assistant, Nurse Practitioner Services

Physical, Occupational, Speech Therapy

Our occupational therapy programs are designed to make the activities of daily living easier and more comfortable for each patient.
